Truffle was 5 months old on 8/13; from pretty early on she could be in her crate for nighttime for 8-10 hours (much longer than suggested by her age).

For a while now, we've been concerned that she never "asks to go out" but for the most part, we are able to take her out frequently enough that we have had few accidents. The accidents that we have had, we attributed to us not paying attention and taking her out.

Well, this morning there was a "puddle" in the kitchen (on tile floors) with the family room door open to our backyard. (Husband and I were indoors doing other chores).

We've come to the conclusion that the only "housebroken" attribute is that we've taken her out to our backyard often enough, or left her outside often enough, that she hasn't had to really learn that peeing in the house is absolutely not OK ...

NOW WHAT DO WE DO?????

Has she been spayed yet?

Luna had some incontinence for a while while she was sleeping if she went to sleep with a full bladder.  It started when she was spayed but resolved itself by the time she was done puppyhood.  It only resurfaced if she had a bladder infection which happened once or twice.

I'd take a step backward; assume she is not solidly housebroken and limit her unsupervised freedom in the house. Keep her tethered to you or crated/gated if you can't watch her, and I wouldn't let her go in and out freely without supervision, or leave her outside on her own, ever. You can't reinforce or correct what you don't see. You need to be out there when she potties outside to praise her (Good potty!!! Good pee-pee!!) so that she understands that she's made the correct choice. Conversely, you need to see an accident happening in order to correct it. And always take her outside through the same door, so that she will go to that door automatically to let you know she has to go out. 

I'm interested in the responses to this. The doodle girls have been the most reliably housebroken dogs I've ever had (small dog problems) but none of them have ever really "asked" to go out. They just know to hold it until we're outside, I guess? We go out frequently, so it's never been an issue. But I'm curious, is everyone else's dog alerting at the door when they need to go out? Are we weird?

Jackdoodle never alerted me, but he was a camel and would "hold it" forever, or  until I decided he needed to go, whichever came first. :) he was trained to potty on command. But he also came to me already housebroken at 14 month old.
My last Poodle always went to the back door when she wanted to go out. Jasper does, too, but in his case, it's because he wants to chase chipmunks. :) He is also turning out to be a camel. 

Luna was definitely a camel too.  Peed maybe 3 times a day, if that.. and when she did it lasted a good 20 seconds lol.

She definitely asked out when she needed to go though, which was mostly in the morning, after work and before bedtime... although at bedtime sometimes she just wanted to bark at shadows or check for marauding racoons :p
