I don't think anyone has mentioned this yet.....do you clean the area where she has accidents with a product like Nature's Miracle which has an enzyme that gets rid of the odor on a molecular level and the dog can not smell it anymore?? It is important to be sure the odor is gone or a dog will return to that spot and "go" again. Even when we can't smell it...a dog can! I wanted to also agree with those who are saying that your pup is still a baby and needs time to mature. She will get there...she just needs time, love, and understanding. She is a cutie!!
One more thought.....can you take her out for a nice long walk during the time she usually has her accidents and let her sniff and pee? Maybe a walk in the neighborhood will do the trick.
Milla is 13 weeks. When we take Milla out to potty, we take her out on a leash and when she is done, back inside we go. That eliminates distractions outside and stays strictly business. There have been times she has gone out and went potty but then in 15 minutes wants out again. So out we go and she goes again. I don't think she went completely the first time. You begin to learn their schedule and communication style. If at 7pm, she needs to go out then out she goes.
