How do you punish her? May she get more attention from this, which she appears to like. Could she have a urinary tract infection? Lots of questions but not any answers yet.

I don't think this has anything to do with her personality. I had one of the most dominant females in the world last time around who wouldn't do anything without a good reason, could care less about pleasing anyone, and she never, I mean never, eliminated in the house from the time she was three months old. Dogs really don't want to soil their own living spaces if they have an option.

Two things:

"Cleaning up" and shampooing carpets will not get rid of urine odors. You may not be able to smell anything, but a dog's sense of smell is up to 10,000 times stronger than yours, and unless you use an odor neutralizer specially designer to eradicate the smell of the urine, like Nature's Miracle, a dog will keep returning to that spot. When you use the neutralizer, you must completely saturate the area; spraying is not enough. It has to get through the carpet and pad down to the floor.

The other thing is to take her in to the vet to be checked for a UTI. 

I lied, three things:

You do not "punish" a dog for making a mistake...ever. You correct them. There is a big difference. I don't know what your punishments consist of, but obviously they are not working. 

How have yu trained her? How does she let you know she has to go- or she just doesn't know how.
Have you ever tried potty bells? I don't think there is communication here.
How do you punish what is wrong when she has no idea what's right ( by telling you)
How about a doggie door option? This way, she can go without trying to figure out how to let you know.

If its not a UTI, then does Ember always pee in the same spot? 

Biba had a favored pee spot, right in front of our patio door. I was bad at catching her in the act so I couldn't yell or startle her. So I did 4 things:

1. Severely limited her time in the room with the patio door. I would hole up in our bedroom with her and she would hold it. I think that was like an extended crate for her as thats where she sleeps.

2. Took her out every 2-3 hours like she was a puppy

3. Used Nature's Miracle as our mopping solution for that area. Even when there was no accident, thats what I used to clean. I assumed that the smell was just buried deep in our floor over there. 

4. Fed her in the pee spot. Dogs don't like to pee where they eat or sleep.

She pretty much stopped within 2 weeks of following the above. Dogs have strange memories so maybe she forgot that that was her favored spot. I don't know for sure. I only take her out 4-5 times a day now (2-3 walks + 2 additional pee breaks).

Good luck! Its frustrating but it will work itself out. I had 1-3 accidents a week till we tried the above at 6 months old and since then its been fine. We have one offs some times but we shrug them off and move on. Definitely get the Nature's Miracle. 

You can not punish ( again, whatever punish is??) after the fact. You must catch in her the act. Your reaction should be not a 'punishment' but rather to rush her outside.
Show her what you want her to do. Don't show her anger/punishment

Please stop using the word punishment, it is highly inappropriate to 'punish' a dog, Perhaps 'correct' it but the word punish is punitive and after the fact. Forgive me if this is just a misuse of a word but it is seriously upsetting me.  Dogs do not understand anything after the fact. To treat any dog like a human is also incorrect, they are dogs. Just 'catch' her in the act, growl in your best low dog voice and rush her straight outside, when she goes, jump for joy! Please do rule out a UTI before anything else.
