We got Meg's at Petsmart and they have a machine right there that engraves them. Her name is on the front, the back has my home and my cell # plus the "has ID chip" on it. I decided to do that because in our area people constantly find dogs and post them as lost but never seem to think to take them to vet or shelter to check for chip.
Be sure to put cell phone numbers on the tag. We had our home phone number which would be useless if Bodi ever got lost while we travel. I have heard that some people put "Needs medication" to stop people from stealing the dog. Most pet stores have the machines. Our vet has one too!
That's a great idea to put "needs medication"...also I've heard NOT to put the name of the pet on the tags. If the dog is stolen, it would make it more difficult for the thieves if they do not know the dogs name. mehttp://www.gundogsupply.com/important-i-d--tag-information.html

We use ones from Boomerang tag http://www.boomerangtags.com/ and love them. We have the dog's name, our last name, my cell number and their microchip number. No address
I put Charlie's name on one side, and on the other my phone number and "If I'm out, I'm lost". I figure it makes no difference if someone knows his name. He loves everybody. I never know if neighborhood dogs are "supposed" to be outside alone, so I wanted to make it clear to anyone that if he's wandering without me, he's in danger. Yes, I know people aren't supposed to let their dogs roam.
They are getting pretty worn out, so I will probably put his microchip # on the tag when I have new ones made.
